Registration Information

    Please refer to the Proof of Residency Check Sheet – Returning Students form below for information regarding what documents will be required to prove residency in North Chicago School District 187.  Documentation of your Mortgage or a complete current signed and dated lease, along with a photo ID are mandatory verification documents and you will not be able to complete verification without this documentation.  Two other forms of proof of residency from the list are then required to support the mortgage or lease document.

    Transfer Information  – If you will not be returning to North Chicago School District, complete the End of the Year Request for Transfer Form and submit it to the main office at your student’s school. High School students should submit this document to the Registrar’s Office. After submitting the End of the Year Request for Transfer, the secretary will prepare an Illinois State Transfer Form and attach any medical documents and final report card for the student. If you owe any fees, you must resolve the balance before you are issued an Official Transcript. When you complete the Transfer Process, you will need to complete the entire registration process should you plan to return to North Chicago School District.

    Current Student Withdrawal Procedures  A student will be removed from the rolls when he/she transfers, the parent completes the Request for Withdrawal Form or is expelled. The student must be accompanied by his/her parent who must have a valid photo ID and report to the Registrar’s office for the High School and main office for Elementary and Middle School. The student must bring all property belonging to the school. This includes text books, library books, etc. The Registrar’s office for the High School and main office for Elementary and Middle School will then give the student a withdrawal card which the student then takes to individual teachers. At this time, the student turns his/her books into the teacher and the teacher signs the card. A student owing money should be prepared to pay balances at the time of withdrawal in order to have official records released.

    The parent/guardian must come into the school and present proper identification to the secretary to verify that he/she is the parent/guardian. The secretary will date and sign the Request for Withdrawal Form, which will coincide with the date the student entered into the computer as the date the student actually withdrew from school. The secretary will release a copy of the student’s transcript, all medical records, and a copy of the form letter from the Illinois State Board of Education indicating the status of the student upon leaving the school.

    A student can be withdrawn from school when another school informs us that the student is in attendance at their school. The school will be required to send us the proper forms filled out. If a student leaves without returning books or owing an outstanding obligation, every effort will be made to collect the outstanding obligation before records are sent out.

    Students Who Are Returning to the District All returning students will complete registration at your school. Returning students must have all registration requirements completed before June 6,2016 to ensure school assignment, class placement, and transportation if needed. Those families who do not complete registration within their school by their school’s registration deadline must go to the District Registrar’s Office located at North Chicago Community High School to complete registration. Summer registration will be done by appointment only for returning D187 students who have not yet registered.

    Summertime Student Registration – The District Registrar’s Office will be open starting June 13, 2016, through July 29, 2016, from 8:00 A.M. – 2:00 P.M. for all new student registration.  Please call (847) 689-6333 for information regarding this process.

    After the first day of school, regular Student Registration hours will be Monday through Friday 9:00 AM to 1:00 PM, excluding holidays and days the district is closed, at North Chicago High School.

    Incoming Kindergarten students – Your child must be 5 on or before September 1, 2016, in order to attend kindergarten in the 2016-2017 school year.  New Kindergarten students who did not participate in Kindergarten Round Up in the spring of 2016 should follow the “New Students to the District” instructions above for registration.

    Transportation– Transportation will be provided if the student lives more than 1.5 miles from the school or has to walk through a Board-approved Hazardous Crossing.  Letters will be sent home early August notifying parents and guardians if the student is eligible for transportation and the bus stop location.  Due to the declassification of several Hazardous Crossings, some students may no longer qualify for transportation.

    School fees Fees may be paid in cash or by money order (made out to North Chicago CUSD 187) and are expected at the time of registration.

    Free and Reduced LunchFor families who feel they would qualify for the Free and Reduced Lunch Program, applications for the 2016-2017 school year will be available at the District office, beginning in August.  Each family completes ONE application for all of their students in the district.  For Students who qualified last year for Free and Reduced Lunch in North Chicago School District will retain their certification for the first 30 days of school.  If we receive verification of your certification for the 2016-2017 school year in August, you will not need to fill out a new application; however, if we don’t receive verification of your certification or you are turned down and you feel you still qualify for this program, you will need to complete a new application for the 2015-2016 school year.  Contact Susan Oglesby, 847-689-8150, if you have questions regarding your certification. Families who fail to complete a meal application will receive a bill for all meals, and/or receive an alternate meal different from the menu items.

    Immunizations – Any student who does not have current, up to date immunization records on file by October 15, 2016 will be excluded from school until such records are obtained.
