District 187 Physical Examination and Immunization Requirements

All students must be up-to-date with physical examination and immunization requirements. All forms must be turned in to the School Health Office before the start of school.

All students entering Early Childhood, Kindergarten, 6th grade, and 9th grade must complete a current physical examination on the approved Illinois Department of Public Health Form. A sports physical does not meet the requirement. A current physical is one that was completed within one year of August 2024. The parent portion on the upper back side of the form must be completed and signed. All students must be up-to-date with the State of Illinois immunization requirements.

EFFECTIVE 1ST DAY OF SCHOOL: STUDENTS MAY NOT ATTEND UNTIL HEALTH REQUIREMENTS ARE MET OR CLEARED BY THE SCHOOL HEALTH OFFICE.

District 187 offers physical and immunization clinics onsite throughout the year. These clinics provide the required school physicals and immunizations for students with Medicaid as their primary insurance. Please get in touch with your student’s school Health Office for information on dates and locations.

Any student who will take prescription or non-prescription medication during the school day will require the Medication Authorization form signed by a healthcare provider to be on file within the health office. A new form must be submitted each school year.

Physical Examination and Immunization Requirements by Grade – English

Preschool

Complete Preschool Physical Examination.

No additional immunization is required if it is up to date.

Kindergarten

Complete Kindergarten Physical Examination and all required immunizations :

  • Four or more doses of Diphtheria, Tetanus, Pertussis (DTP or DTaP) vaccine, with the last dose being a booster and having been received on or after the 4th birthday. 

  • Four doses of the Polio vaccine, with the last dose being a booster and having been received on or after the 4th birthday. 

  • Two or more doses of the Measles, Mumps, and Rubella vaccine, with the first dose received on or after the first birthday. 

  • Two doses of the Varicella vaccine 

Complete Kindergarten Dental Examination – Must be turned in before May 15, 2025.

Complete Kindergarten Vision Examination – Must be turned in before October 15, 2024. Waiver form for Eye Examination Report

1st Grade

No additional physical/immunization requirements if up-to-date.

2nd Grade

No additional physical/immunization requirements if up-to-date.

Complete 2nd grade Dental Examination – Must be turned in before May 15, 2025.

3rd Grade

No additional physical/immunization requirements if up-to-date.

4th Grade

No additional physical/immunization requirements if up-to-date.

5th Grade

No additional physical/immunization requirements if up-to-date.

6th Grade

Complete 6th grade Physical Examination and all required immunizations including:

  • One dose Tdap vaccine at age 11 years or older, regardless of interval since the last dose of DTP, DTaP, or Td.  

  • One dose of Meningococcal Vaccine (meningitis)

  • Two doses of Varicella Vaccine (chicken pox)

  • Three doses of Hepatitis B at the required intervals

Complete 6th-grade Dental Examination – Must be turned in before May 15, 2025.

7th Grade

No additional physical/immunization requirements if up-to-date.

8th Grade

No additional physical/immunization requirements if up-to-date.

9th Grade

Complete 9th grade Physical Examination and all required immunizations.

Complete 9th Grade Dental Examination – Must be turned in before May 15, 2025.

One dose of Meningococcal Vaccine

10th Grade

No additional physical/immunization requirements if up-to-date.

11th Grade

No additional physical/immunization requirements if up-to-date.

12th Grade

Must show proof of receiving two doses of meningococcal conjugate vaccine (meningitis), with the second dose administered on or after the child’s 16th birthday and at least 8 weeks after the first dose. The second dose is unnecessary if the first dose is administered on or after the child’s 16th birthday.

Illinois Sports Pre-Participation Examination

The Illinois Pre-Participation Examination form is required for students planning to play or try out for sports. This form does not replace the Illinois Physical Examination form; it is required in addition to any Physical Examination form and is valid for 395 days (approximately 13 months) from the examination date listed on the form.
