The Illinois School Code (105 ILCS 5/26-1 et seq.) states that children between the ages of 7 and
17 must attend public school; however, an exception is made for “…..any child attending a
private or parochial school where children are taught the branches of education taught to
children of corresponding age and grade in the public schools, and where the instruction of
the child in the branches of education is in the English language.”

Complete all information on the form, and submit the form in the following ways ONLY:
Elementary School – Send it to school with your child at least 48 hours in advance, or deliver it in person to the main office of your child’s school at least 48 hours in advance. Student withdrawals, just like registration, must be done in person. You will not receive a transfer without completing the withdrawal procedure. This form is also available in every school office.
If your student is in middle school or high school this form MUST BE COMPLETED IN PERSON WITH A VALID ID.
Middle School students must go to the Main Office and get a Student Drop Card to be completed on their last day of school. Once that card is completed by each teacher, all books/materials are returned, and the completed card is returned to the Main Office, you will then be issued a transfer and a copy of the current grades.
High School students must go to the Guidance or Registration Office and get a Student Drop Card to be completed on their last day of school. Once that card is completed by each teacher, all books/materials are returned, and the completed card is returned to the Guidance or Registration Office, you will then be issued a transfer and a copy of the current transcript.
REGISTRATION HOURS ARE 9 A.M. – 1 P.M.
PLEASE BE AWARE, ALL RECORDS, TRANSFER AND OR/ TRANSCRIPT REQUESTS WILL TAKE 3 – 5 DAYS.
THERE WILL BE NO EXCEPTIONS.
